This is really one of the most exciting part of the whole SEO operation - KEYWORDS/KEYPHRASES. I follow some ways to decide KWs/KPs for my projects:

1. Studying the targeted domain , sector, audiences, demography, services, etc related to the website and then brainstorming the best list of keywords/keyphrases, best suited for the website. Extensively researching and analyzing them and then coming up with the best set of them.
   
2. Checking out Google Analytics for all the keywords/keyphrases which are bringing in traffic to the website.
   

I follow the first step for a new website and the second one for an old one. When a website is new a set of 5 best keywords, depending on their avg. monthly searches and competition, is being targeted for on-page and off-page SEO.

When a website is old, each page is optimized as per the Google Analytics' KWs/KPs record, which shows that which particular keywords/keyphrases is bringing search engine's organic traffic to each webpage.
