Looking forgraphic Product list addon

I'm looking for an add-on that will help me with galley of products that'll shown as a galley with text labels.
Clicking a product will open a pop-up form with some questions for the user to fill, and then it'll send it to me via e-mail.
The most important thing here is good UX, and the ability of right-to-left orientation.

Any suggestions?
