Travel / medical - worth it?

Hi all,

Considering that affiliation is seen increasingly worse by google, look for other alternatives to supplement my income. I want to make one or two niche sites on medical tourism and, both internationally. People working in these niches I can tell if they, if they deserve? That in terms of adsense site.

I can not do tests. I do not want some exact figures, I want to know how it works in general.
