Tools or Ways to Measure Social Signals

I'm sure there must be a tool that to do this

I have a wordpress blog and added social media buttons to all the pages (google+, facebook, twitter, linkdin..etc)

So what I'd like to do is measure on a weekly basis, how many social interactions we get right across the site. Totalling up the FB likes, the Google pluses, twitter followers... etc and etc.

Any ideas of a tool that I can use?

Any tips most appreciated!
