Amazon affiliate disclaimer?

I am joining Amazon Associates, I am planning to promote products on my blog, Where will you put affiliate disclaimer on your blog or website? Is it OK to have this disclaimer inside a page on my website titled 'Affiliate disclaimer'? Or do I need to have the whole statement visible on every page that has an Amazon link?

I have mine on the footer of every page that contains an Amazon link.

I've got the Amazon Affiliate disclaimer on the sidebar of every single page of my website, and a generic affiliate disclaimer before every blog post which also links to my actual disclaimer page.

I have heard of people having their Amazon Affiliate accounts closed without warning for not having the disclaimer in an obvious location.

I would recommend having it either in the sidebar or in the footer of every page of your website, even if some of the pages don't have any affiliate links on them.

It's better to be safe than sorry, especially with the earning potential that Amazon has.
